Adult Case Manager (Community Service Program)
A great opportunity to apply and refine your clinical skills while treating adults with severe and/or persistent mental illness in the context of a multidisciplinary treatment team. Deliver psychotherapy to individual adults, couples and families. Experience in short term therapy. We are seeking a self-motivated seasoned clinician to work 32-40 hours per week. Supervision provided.

Director of the Community Support Program
This position manages the CSP Treatment Team in both Laconia and Plymouth, as well as the Supported Housing and Supported Employment programs. Works very closely with the medical staff who support the team in the delivery of services.   Responsible for managing the recruitment, hiring, and performance evaluation of the staff within the program.

Adult Services Team Coordinator (Plymouth)
Oversees an interdisciplinary team that provides services to eligible consumers in the Community Support Program. Provides regular supervision with a clinical and administrative focus in an effort to ensure best practice standards are met. Oversees caseloads of staff and utilization of services to maintain clinically appropriate/indicated care. Assures quality and timely documentation on records/paperwork of staff on their teams according to established standards. Provides backup case management and crisis intervention to clients as needed. With the Adult Services Director, is responsible for the recruiting and professional development of the team staff. Assists in developing the program’s budget and monitors financial and statistical data to manage the program’s costs, revenues, and overall service delivery. Is involved in pursuing resources and methods that will improve the program’s viability.

Case Manager (Children & Families)
Provide individual, group and family therapy through a case management service model. Develop and implement with families, individual service plans. Provide home based services to children and families. Facilitate community integration and collaboration. Provide assessment, monitoring, support and crisis intervention. Fulfills productivity and documentation requirements.
