Works very closely with the medical staff who support the team in the delivery of services. Responsible for managing the recruitment, hiring, and performance evaluation of the staff within the program. |

| Adult Services Team Coordinator (Plymouth) |
| Oversees an interdisciplinary team that provides services to eligible consumers in the Community Support Program. Provides regular supervision with a clinical and administrative focus in an effort to ensure best practice standards are met. Oversees caseloads of staff and utilization of services to maintain clinically appropriate/indicated care. Assures quality and timely documentation on records/paperwork of staff on their teams according to established standards. Provides backup case management and crisis intervention to clients as needed. With the Adult Services Director, is responsible for the recruiting and professional development of the team staff. Assists in developing the program’s budget and monitors financial and statistical data to manage the program’s costs, revenues, and overall service delivery. Is involved in pursuing resources and methods that will improve the program’s viability. |

Provide home based services to children and families. Facilitate community integration and collaboration. Provide assessment, monitoring, support and crisis intervention.
